Providers interested in becoming part of the YOUnity Medical Affiliate IPA network should fax the following information to the Contracting Department at (626) 458-8051:

• Letter of Interest - please include scope of services, hospital/facility privileges
• Curriculum Vitae
• W-9

We will respond to all interested providers within thirty (30) days.

Demographic updates, such as practice location as well as phone and fax number updates, should be submitted in writing via fax to the Contracting Department at (626) 458-8051.
